The Community Health and Wellness team has a variety of resources and programs available for Lancaster residents ranging from nutrition-related services to safety-related services, and we’ve highlighted information around these key programs below.

Food Farmacy Program
The Food Farmacy Program connects individuals to counseling with a dietitian and provides access to healthy food options. The goal of this program is to assist patients dealing with medical conditions by supplying them with healthy food options that work to improve their health and well-being. Patients enrolled in the program have seen success with reported increases in healthy eating habits, improved blood pressure and weight loss.

The program is eligible for patients experiencing food insecurity with select medical conditions. To learn more about Food Farmacy and the eligibility requirements, please click here.

Healthy Homes Program
The Healthy Homes Program works to create healthy communities by minimizing home health hazards. The program distributes educational materials around hazards such as mold, radon and household pests, and also provides free home assessments by trained specialists. Select homeowners also have the opportunity to address their home’s hazards when meeting criteria based upon their location, household income and build of the home. Families that meet the criteria for this program work with a Healthy Homes Specialist to complete the application process, have their home’s health assessed and secure any necessary repairs.
